Release of Liability and Waiver, Assumption of Risk and Indemnification

 

I, acknowledge that I wish to participate in training with Artistic Athletes, and attest that I have read, fully understand, and agree with the following representations and conditions:

I am aware of the risks of participating in the program/activities, and assume all risks, foreseeable and unforeseeable, in any way connected with my participation herein. I agree not to institute any claims or legal action against Artistic Athletes LLC for any claim released by this Agreement.

I am voluntarily participating in a class or classes provided by Artistic Athletes LLC in person or virtually. I will be receiving instruction and information concerning fitness and wellness techniques, which may include weight training and other physical activities. I represent and warrant that I have no physical or mental health condition that would prevent my safe participation in these classes. I acknowledge and agree that Artistic Athletes LLC is not a medical professional and does not provide any medical diagnoses or treatments. I agree that if I have any medical condition, I will seek the help of a medical professional.

 

I agree to protect, defend, and indemnify Artistic Athletes LLC and any of its employees or owners in regards to any claim or expenses (including attorneys’ fees, claims by a third party or otherwise, that might be made on my behalf) in any way connected with a claim related to my participation in the program/activities. I have read and fully understand, agree to, and accept voluntarily all provisions of this Release of Liability and Waiver, Assumption of Risk, and Indemnification.

 

In the event that the participant is under the age of consent (18 years of age) then this release must be accepted by a parent or guardian.
